breast cancer?

Minggu lepas saya berada di sub-department Surgical Oncology. Oncology bermaksud the branch of medicine that deals with tumors, including study of their development, diagnosis, treatment, and prevention. Atau secara ringkasnya adalah tentang barah @ cancer.

Saya berpeluang melihat dan membuat pemeriksaan ke atas beberapa cancer patients. Mostly breast cancer, thyroid cancer, parotid gland tumor, and skin cancer.

Kebetulan, semalam saya berborak dengan seorang kenalan baru yang telah kematian ibunya 2 tahun yang lepas disebabkan breast cancer. Dengan ilmu dan pengalaman baru yang saya dapat, saya merasa tergerak untuk share sedikit ilmu tentang Breast Cancer terutama untuk wanita.

The breasts are composed of fatty tissue that contains the glands responsible for milk production in late pregnancy and after childbirth. Within each breast, there are about 15 to 25 lobes formed by groups of lobules, the milk glands. Each lobule is composed of grape-like clusters of acini (also called alveoli), the hollow sacs that make and hold breast milk.

The lobules are arranged around ducts that funnel milk to the nipples. About 15 to 20 ducts come together near the areola (dark, circular area around the nipple) to form ampullae - cavities that store the milk before it reaches the nipple surface. Montgomery's glands are small oil glands that are located around each areola. They release a lubricant that protects the nipples during nursing.

The breasts are not always exactly the same size or shape. They are incompletely developed at birth and - in men - remain small and undeveloped unless subjected to abnormal hormonal stimulation. In general, breast formation is complete within a year or two after the start of menstruation; however, the acini keep growing, and fibrous and fatty tissues are continually added during adolescence. Pregnancy and nursing cause further increases in breast size. As a woman ages, the fatty tissue of the breasts may become more prominent than the glandular tissue, and the breasts may feel softer. The breasts gradually atrophy (shrink) after menopause (the end of menstruation).

The breasts cover a large part of the chest wall. In front, the breast tissue may extend from the clavicle (collarbone) to the middle of the sternum (breastbone). On the side, breast tissue may continue into the axilla (armpit) and reach as far as the latissimus dorsi (muscle extending from the lower back to the humerus bone of the upper arm).

Lymph is a clear, tan fluid that contains lymphocytes (white blood cells that fight disease). Lymph is drained from the breast tissues by a rich supply of vessels. Such lymphatic vessels connect with a network of lymph nodes that are located around the breasts' edges or in nearby tissues of the armpits and collarbone. The breasts' lymph nodes are not linked in a straight line. Instead, they are staggered and fixed within fat pads - an arrangement that complicates lymph node removal during breast cancer surgery.

Lymph nodes play a central role in the spread of breast cancer. The axillary (underarm) lymph nodes are particularly important, as they are among the first places that cancer is likely to be found if it metastasizes (spreads) from the breast. This lymph node cluster is often referred to as the 'tail,' or level I nodes. Level II nodes are located underneath the pectoralis minor muscle, and level III nodes are found near the center of the collarbone.

Tumor terbahagi kepada dua : Benign (jinak) dan malignant (ganas). Maksudnya benign tu masih di tahap awal dan boleh diubati, dan malignant tu dah masuk terminal stage yang susah untuk kembali ke fungsi normal.

Kalau di Indonesia, kebanyakan pesakit lambat mendapatkan rawatan sebab tidak ada duit untuk bayar biaya perubatan. Jadi biasanya mereka datang bila gejala-gejala yang sangat teruk dah timbul. Waktu tu dah agak terlambat untuk diubati. Kalau di Malaysia saya kurang tahu, tapi saya rasa patient lambat datang mungkin sebab malu untuk diperiksa, atau tak ambil kisah sangat.

Tanda-tanda awal kanser payudara adalah timbulnya benjolan yang agak keras pada payudara. Kalau nak check sendiri, bolehlah periksa sendiri ada atau tidak benjolan tersebut terutamanya pada hari ke 5-7 menstruation. Kalau ada, make sure jangan tekan benjolan tersebut, dan segera ke doktor.

Kembali pada cerita kenalan baru saya tadi, waktu arwah ibunya pertama kali detect benjolan tersebut, benjolan tersebut ditekan sampai hilang. Sebab waktu tu benjolan masih kecil. Bila benjolan tu hilang, mesti ramai yang ingat benjolan tersebut dah tak ada, dan tumor tersebut dah hilang.

Tapi, fakta sebenarnya adalah, benjolan tersebut walau sekecil mana pun, adalah suatu mutated cells. Sel-sel yang telah bermutasi tidak akan sama dengan sel-sel normal yang lain. Sel tersebut harus dibuang dari badan supaya tidak membesar dan menyebabkan mutasi dari tisu-tisu di sekelilingnya.

Bila benjolan tersebut ditekan, dan hilang, itu tidak bermaksud tumor dah tak ada. Sebaliknya, tumor cells adalah sangat mudah rapuh, dan bila ditekan akan menyebabkan sel-sel tumor tersebut masuk ke aliran darah. Bila dah masuk ke aliran darah, as we know, aliran darah kan mengalir ke seluruh tubuh. Jadi darah akan membawa sel-sel barah tersebut untuk migrate lebih jauh dari breast. Dan ini memudahkan terjadinya metastasis (The process by which cancer spreads from the place at which it first arose as a primary tumor to distant locations in the body)

Sedangkan, the primary pathway untuk penyebaran breast tumor adalah melalui lymphatic system. Nampak tak dalam gambar kat atas, ada nodes warna hijau yang banyak2 tu? yang macam biji-biji kacang tu? That is the lymphatic system. Saya pasti semua orang ada belajar tentang lymphatic system masa Biology SPM dulu. Bila breast tumor tu dah merebak, tempat pertama yang akan dia pergi adalan pada lymph nodes tersebut. Jadi, tandanya adalah benjolan di bawah ketiak, di atas dn di bawah clavicle. Kesimpulannya, mana-manalah tempat yang ada benda macam kacang warna hijau dalam gambar tu.

Kenapa benjolan tersebut boleh sebabkan seseorang wanita meninggal dunia?

Dulu saya terfikir jugak, kenapa breast cancer, kalau dah besar, boleh sebabkan orang mati. Sedangkan benda tu kat breast. Sekarang dengan sendirinya saya dah dapat jawapan tersebut.

Tempat paling dekat untuk breast tumor spread adalah LUNG and BONES. Tempat lain banyak lagi, tapi tempat utama adalah lung & bones. Biasanya, kalau dah masuk lung, or spaces kat luar lungs, cancer tersebut dah masuk stage 4.

Bila dah masuk lung, secara automatik, symptoms of penyakit paru-paru pun akan timbul. Yang menyebabkan kematian adalah sesak nafas. Bila spaces di luar lung (pleural space) tu dah dimasuki sel-sel cancer, pleural fluid akan bertambah, dan lung akan susah untuk expand. Inspiration and exhalation process terganggu. Less oxygen akan sampai ke lung, dan less oxygen akan dibawa ke seluruh badan. Bila badan kekurangan oxygen, faham-fahamlah apa yang akan berlaku seterusnya kan. Oxygen adalah sangat penting untuk setiap sel-sel dalam tubuh.

tq 4 sharing..is there any prevention to dis cancer??i wanna avoid dis..afraid.....dun wanna suffer due to dis problem...
